24-Hour pH Impedance Study to Evaluate Reflux Disease
est. $ 148

For those who are suffering from an acid reflux that typically results in chest pains and a burning sensation in the throat, the doctor may recommend a 24-hour pH impedance study. During this exam, a catheter that is equipped with an acid-sensitive tip is passed through the nose and settles in the esophagus. The tube is connected to a device that monitors the level of acid that travels to the esophagus. After 24 hours, it is removed and the tip evaluated. A personalized treatment program is then provided based on the result of the test. Acid reflux can be managed through simple diet and lifestyle changes and by taking medications including muscle relaxants, blood pressure drugs, and anti-inflammatory pain relievers.

This is service is for patients who require medical assistance after office hours, which is after 5pm during weekdays. This could be an initial consultation where symptoms that are being experienced by the patient are discussed or, standard diagnostic tests are performed to identify the causes of these symptoms. It could also be a follow-up consultation for the results of the previous tests or to check whether or not the patient is recovering nicely from a previous procedure or surgical treatment.

Colonoscopy

Colonoscopy is performed to examine the linings or walls of the large bowel and rectum to check if there are lesions and/or polyps and to confirm the presence or absence of colon cancer. During the exam, the patient is sedated while a colonoscope goes through the anus. The camera that is found at the tip of the probe then sends live videos of the linings to a computer monitor that allows the doctor to assess or identify polyps. In many cases, these are removed during the same procedure or tissue samples are collected for a biopsy. As a form of preparation, the doctor will put the patient on a soft diet while the colon will be cleansed using laxatives a few days before the actual procedure. The goal is to empty the colon so the diagnosing physician will be able to make an accurate diagnosis.

Diagnostic and Therapeutic Upper and Lower Gastrointestinal Endoscopy

While several imaging tests can provide some basic idea of the conditions of the internal organs, the doctor may still require a better visual image using a probe known as an endoscope (which is equipped with both a light and camera at the tip that illuminate the lining and send live feed to a computer screen) to make or confirm a diagnosis. Because the tube is long, flexible, and narrow, it’s the ideal diagnostic tool for the upper and lower gastrointestinal (GI) tract, including the liver, stomach, esophagus, small and large intestine, and the rectum. The endoscope is also used to collect tissue samples for a biopsy or remove polyps to minimize or eliminate the risk of cancer.

Dr. Pho Choo Hean offers office hour emergency consultation for specific cases that require immediate medical attention, which include caustic ingestions, food impaction, and acute liver failure. The same type of medical attention can be given if the patient is suspected of suffering from nonvariceal upper GI hemorrhage, acute pancreatitis, or if certain complications have developed following a procedure such as endoscopy. This type of consultation is offered from Monday to Friday from 8:30am to 5:00pm and Saturday from 8:30am to 12:00pm.

Esophogeal Manometry

Commonly referred to as Esophageal Manometry, High-Resolution Manometry (HRM) is a simple and quick test to determine whether the esophagus is functioning properly. This is often carried out when a person experiences certain symptoms such as heartburn, acid or gastric reflux, swallowing difficulty, and chest pain. During the procedure, a local numbing medicine is administered to minimize the pain. A pressure-sensitive tube is then introduced into the nose and is guided toward the stomach, after which it is pulled back to the esophagus. Different exercises such as swallowing are then performed to monitor the function of the organ.

Evaluation and Treatment of Diseases Related to Gastrointestinal Tract

The gastrointestinal tract is composed of the upper and lower section that include the oral cavity, salivary glands, stomach, duodenum, liver, and esophagus secretory glands. A disease affecting any of these parts may also result in a problem in the entire tract. Thus, an evaluation of both tracts is necessary so doctors can make an accurate diagnosis and later on, provide a more accurate and effective treatment plan. While the most common diagnostic methods include blood tests and imaging exams such as CT and MRI, imaging with probes such as a colonoscope and oesophageal gastroduodenoscope are more preferred by doctors. Treatments depend on the findings, which may include surgery, medications, customized diet, and stress management.

Evaluation and Treatment of Liver-Related Diseases

As one of the biggest and vital internal organs of the body, the liver should be checked from time to time for any diseases especially if a person develops symptoms such as jaundice, loss of appetite, unexplained weight loss, nausea, pain, and vomiting. Diagnostic exams include function and blood tests, as well as a laparoscopy. It is carried out by creating tiny incisions in the abdomen and inserting a probe that sends live images of the organ to a computer screen. If a biopsy needs to be performed, a tissue sample usually from a lesion is collected and sent to the lab for analysis.

Repeat consultation is also applicable for patients who have received treatment and when specialists at the clinic would want to make sure that the wound is healing nicely and that no complications have developed. Oesophageal Gastroduodenoscope or OGD is a simple and quick diagnostic procedure that involves inserting a scope that goes through the esophagus, stomach, and duodenum (a part of the intestine), all of which are part of the upper GI tract. The test can determine if there are lesions, irritations, infections, or cancers causing gastrointestinal symptoms. Local anesthesia is necessary to minimize the discomfort and pain as the tube is passed through the mouth. The scope, which includes a camera, provides real-time visual data to a computer monitor. The results are used to make a diagnosis and later on, in identifying the best treatment or management option to treat the condition.

Scope to Insert PEG (Feeding Tube)

In cases when a person is not capable of ingesting food and water the natural way (due to sedation or a previous injury or surgery), doctors resort to the use of a PEG (Percutaneous Endoscopic Gastrostomy) tube, which is attached to the abdominal wall and into the stomach to serve as a substitute pathway. To perform the procedure, an endoscope is inserted into the throat all the way to the stomach. Once the exact site for the PEG tube has been determined, a needle is used to puncture the abdomen so the tube can be inserted. The end of the tube is then sutured to the abdomen’s skin.

Cancer screening for colon, stomach, liver, and esophagus may be performed through different tests that inspect both the upper and lower GI tract. One of the most popular procedures is called an endoscopy wherein a long but flexible and thin scope with a camera and light is inserted into the mouth for the upper GI and the anus for the lower GI (colonoscopy). This device transmits live videos of the organs and can also be used to get sample tissues for biopsy. Other tests include imaging scans like MRI and PET, as well as cholangiopancreatography and laparoscopy.

Small Bowel Capsule Endoscopy

A Capsule Endoscopy is a painless way of assessing and monitoring the middle component of the small intestine including the jejunum. It requires a patient to swallow a regular sized pill that contains a video camera. As it passes through the digestive tract, the micro camera snaps images of the linings, which are then sent to an external device that store them. The procedure is completed within 8 hours, after which the images are downloaded onto a computer and analyzed by the diagnosing physician. Although simple, preparation such as fasting and GI cleansing are necessary.
